Plethodon dorsalis is the scientific name to your Easter Zigzag Salamander. It is most likely Wild Caught.
I'm not surprised that your Sally didn't want to eat those pellets. Despite what the label and the pet shop would tell you, most amphibians won't recognize that as food. It's junk, and I would return it to the pet shop or throw it out.
Amphibians require live, moving food. They won't recognize anything that is pre-killed, or premade diets. All prey items should be appropriately sized, not too big or too small.
All feeders should also be captive bred, avoid feeding it insects from outside as they could be contaminated with parasites or harmful chemicals.
